This includes the updated physical form (page 4), parent permission form, concussion awareness form, and registration for your sport. Athletes who do not complete these forms by the deadline will not be allowed to practice or compete until everything is submitted. Bound Sports Participation Help Directions The last day to check out library books will be May 14. Please have all library books returned by May 23. Kristy Molencamp, our CCTC, will be here today, Wednesday, May 7th, all day in the conference room if you need to get signed up for fall semester NIACC classes or have questions.
